Company D, 1st North Dakota Volunteer Infantry, U.S. Army

On May 13, 1899, at San Miguel de Mayumo, Philippine Islands, an entrenched enemy force of 300 men opened fire upon Private Jensen and 11 other scouts. The scouts promptly charged the enemy trenches, routing their forces and capturing the bridge to San Miguel in a sharp firefight. The scouts entered the town and took refuge in a church bell tower. From there they held the town, having driven the enemy entirely out of the city, until two infantry companies arrived that afternoon to help secure the city.
