This example, Serial No. 67-16132, was delivered to the army in June of 1968 and went directly into Action in Vietnam where it saw service which included two crashes due to mechanical failures.
In September of 1970, 16132 was assigned to the Army Aviation School at Fort Rucker, Alabama, where it served for three years before going to Sharpe Army Depot, California, where it remained in storage until mid-1983. It was then transferred to the Utah Army National Guard. It served with the Attack Helicopter Troop of the 163rd Armored Cavalry Regiment in West Jordan, Utah, until March of 1990.
