Dr. Horn, wife Katie, and daughter Reggie Hazel lived in their Putnamville home north of Highway 40 on the east side of the Putnamville Greencastle Road. They were members of the Putnamville United Methodist Church.
Dr. John's 1884 office was later used as a post office before falling into disrepair. It was donated by the McClure family to be used as a museum and in 2004 was moved across Highway 40 o the Putnamville Public Square.
The project was inspired by Georgia Sublette and Ruby Stringer and was accomplished by numerous volunteers under the direction of Lee Stewart.
