It can be reached from Des Plaines Avenue north of Roosevelt Road (Illinois Route 38). The marker is in Section 20 of Forest Home Cemetery. It can be accessed by following the signs to the bridge over the Des Plaines River and then staying straight after you reach the bridge. Touch for map. More about this memorial. Many of the veterans listed here seem to be buried nearby in this veterans section of Forest Home Cemetery, while others are known to be buried elsewhere in the cemetery. At least a handful more Civil War veterans known to be buried at Forest Home are not included on this list.

In addition to this memorial, Forest Home is home to memorials for two different posts of the Grand Army of the Republic. Phil Sheridan Post 615's memorial is about 500 feet north of here in section 17. Its memorial also doubles as the gravesite for one of its members, Wilbur Crummer, whose name is included on this plaque. The other post with a memorial at Forest Home is Columbia Post 706, in section 43. While it's only about Ό-mile northwest of this location as the crow flies, the bridge over the Des Plaines is closed as of January 2025, and the only way to access it from here is to leave the cemetery and re-enter it on 1st Avenue.
